We accept Credit card and Debit card payments. Please, contact us for information about our payment plans. Payment is due and must be posted prior to appointment time. Fees for diabetes education, chronic kidney disease education (stage 4), nutrition counseling and weight management may be covered by Medicare and most other health insurance plans. Please, contact your insurance carrier about coverage questions. A written referral from your healthcare provider is required to assist with insurance coverage.
