We collaborate with healthcare facilities to provide diabetes education and management to patients with diabetes for a better clinical outcome. Our services include:
- an initial assessment of current practices to identify the needs of the facility
- development of evidence-based policies and protocols including hypoglycemia and hyperglycemia protocols
- educational activities geared towards implementation of written protocols and guidelines and diabetes care
- diabetes self-management education to patients and their families to ensure a safe self-care post discharge
- collaboration with other healthcare providers to ensure continuity of care..
Diabetes is a condition associated with serious chronic complications can be prevented or delayed with adequate glucose control. Diabetes is also associated with acute complications like hypoglycemia and hyperglycemia crisis that result in frequent emergency room visits and hospital readmissions. According to the CDC, in 2012 the estimated total cost for diabetes in the U.S was $245 billions with $176 billions accounting for direct medical cost. Diabetes self-management education have been known to reduce diabetes-related emergency room visits.
We will partner with health insurance plans to provide diabetes self management education and/or diabetes care management to your members with newly diagnosed diabetes, uncontrolled diabetes, frequent diabetes related readmissions and emergency room visits.
